Abstract [en]

Welfare state specialisation and fragmentation create organisational gaps where individuals with complex needs risk being caught between authorities and services. Coordination associations have emerged as a solution for financial coordination and collaborative work-oriented rehabilitation. This qualitative case study explores the guide’s role, practices, and significance in this landscape, analysing structural conditions and challenges. Based on interviews, observations, focus groups, and document studies, the findings show that the guide acts as an intermediary and integration engine, bridging fragmented welfare systems through team-based, relational work and cross-boundary resource mobilisation. The study highlights the importance of governance structures, trust-building, and flexible practices to achieve coherent rehabilitation processes.

Specialiseringen och fragmenteringen av välfärdsstaten skapar organisatoriska luckor där individer med komplexa behov riskerar att hamna mellan olika myndigheter och tjänster. Samordningsförbund har vuxit fram som en lösning för ekonomisk samordning och samarbetsinriktad rehabilitering. Denna kvalitativa fallstudie undersöker guidens roll, praxis och betydelse i detta sammanhang och analyserar strukturella förutsättningar och utmaningar. Baserat på intervjuer, observationer, fokusgrupper och dokumentstudier visar resultaten att guiden fungerar som en mellanhand och integrationsmotor som överbryggar fragmenterade välfärdssystem genom teambaserat, relationsinriktat arbete och gränsöverskridande resursmobilisering. Studien belyser vikten av styrningsstrukturer, förtroendeskapande och flexibla metoder för att uppnå samstämmiga rehabiliteringsprocesser.
